We are seeking a compassionate and experienced Community Visiting Nurse specializing in Palliative Care to join our dedicated healthcare team. This role involves providing high-quality, patient-centered care to individuals with serious illnesses in their homes, focusing on symptom management, pain relief, and emotional support. The ideal candidate will work closely with patients, families, and an interdisciplinary team to enhance quality of life and ensure comfort in alignment with the patients goals and wishes Acclaim Health announces the availability of one (1) Regular Part Time Registered Nurse position to work days starting at 8:00am in the Halton Community and Clinics with a focus on Burlington Community . This position requires the individual to be available to work 2 shifts per week and 1:3 weekends including an On-call rotation in addition to the statutory holiday rotation. Pay Rate: $34.76 - $37.64 per visit $35.90-$40.47 PRIMARY FUNCTIONS: * Provide comprehensive nursing care to palliative patients in their homes within the Halton area. * Organizes the day, set priorities based on continuity of care, the type of care to be given and the client’s health needs. * Develops, implements, evaluates, and modifies the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes. * Assesses the health status of the client. * Administers IV therapy including central line and peripheral line maintenance. * Administers medications topically through natural body orifices, through tubes and by injections. * Handles wound care, acute and chronic, for certain types and stages of wounds. * Responsible for ensuring that the lap top is synchronized with the network at a minimum of twice a day, at the beginning and end of your shift, in order to receive information on and update client records. * · Attends team/POD meetings as required. POSITION REQUIREMENTS: * Minimum 1 to 2 years clinical practice in a community environment preferred * Minimum 1 to 2 years of nursing experience preferably in palliative care, hospice, or home health. * Current registration with the CNO * Certification in Palliative or hospice care (preferred) * Current CPR certificate * Excellent interpersonal, communication and teaching skills * Valid driver’s license and access to a vehicle Must be legally eligible to work in Canada Acclaim Health is an equal opportunity employer and is actively working towards a more inclusive, diverse and equitable world.
