Position Details: St. Josephs Hospital Main and Children Location: Tampa, FL Shift Hours: M-F 8am-5pm Status: Full Time, Exempt: Yes The Manager Hospital Care Coordination: Provides overall management of the department team members. Monitors length of stay and identifies way to reduce length of stay through case management and identification of safe alternatives to inpatient care. Promotes and supports BayCare strategic directions and goals (including but not limited to reducing hospital readmission, observation length of stay, patient experience, and team member as customer). Collaborates with ancillary departments, community agencies, and administration to achieve efficiency and effectiveness throughout the continuum of care. Provides leadership for disease management, and care management processes, including identification of target populations, review of evidence-based guidelines, development of protocols/interventions and evaluation of outcomes. Formalizes care coordination processes, roles, responsibilities, policy, and procedures and continuously updates documents as improvements are implemented. Oversees regulatory requirements. Partners with highly specialized clinical programs, including the Left Ventricular Assist Device (LVAD), Transplant, and LifeLink programs, to facilitate complex patient care coordination, ensure compliance with program-specific requirements, and support timely transitions of care. Collaborates with multidisciplinary teams to coordinate organ donor referrals, transplant evaluations, transplant candidate management, post-transplant discharge planning, and specialized post-acute care arrangements, ensuring continuity of care for these medically complex populations. Minimum Qualifications: Preferred Certification: Case Manager Certification (CCM) OR Case Management Certification (ACM) OR Case Management Administrator Certification (CMAC) OR Nursing Case Management Board Certified (CMGT-BC). Education Required : Bachelor's in Nursing or Healthcare Management. Or Master's in Social Work, Mental Health Counseling, Community Counseling, Human Services or Masters in Related Field. Experience Required : 4 years in Acute Care, Case Management, or Social Services and 3 years in leadership role in a related field.
