Manager, Community Health & Wellness Permanent, Full-time Reports to Director, Transitions and Integrated Care POSITION SUMMARY: The Manager, Community Health and Wellness, plays a critical leadership role in advancing CANES’ commitment to high-quality, client-centered home and community care for seniors. This position is responsible for overseeing interdisciplinary roles —including Social Work, Nursing, Nurse Practitioner, and Recreational Therapy—ensuring the delivery of coordinated, holistic services that support seniors’ independence, dignity, and quality of life. The manager will operationalize the team to support clients across CANES programs, including developing internal referral pathways. The Manager will foster collaborative partnerships with external providers to advance the mission of CANES, and healthy aging in general. This position will leverage data to drive continuous quality improvement and monitor performance for informed decision-making. KEY RESPONSIBILITIES: Integrated Care & Service Coordination Accountable for ensuring programs are meeting MSAA targets or organization/funder targets in accordance with service delivery. Lead the development and implementation of integrated care models that address seniors’ physical, cognitive, emotional, and social needs. Ensure effective coordination across programs to support seamless service delivery in home and community settings. Establish and maintain clear internal processes and referral pathways to ensure timely access to services across CANES programs. Promote person-centered assessment and care planning, aligning services with client goals, preferences, and cultural needs. Leadership & Team Oversight Provide leadership, coaching, and supervision to the interdisciplinary team (including Social Work team, Nursing staff, and Recreation Supervisor). Responsible for quarterly and annual performance management of the team. Foster a collaborative, inclusive, and client-centered culture grounded in CANES’ values of compassion, respect, and excellence. Support recruitment, onboarding, performance management, and professional development of staff. Ensure optimal staffing, scheduling, and resource utilization across programs. Data, Performance & Quality Improvement Ensure staff are documenting professional and regulatory standards. Oversee the use and enhancement of Power BI dashboards to track program performance, service utilization, and client outcomes. Analyze data to identify trends, service gaps, and opportunities for improved client care and operational efficiency. Develop and present regular performance reports to senior leadership. Lead continuous quality improvement initiatives aligned with best practices in seniors’ care and community health. Program Development Integrated Initiatives Guide the design, implementation, and evaluation of integrated, recreation, and supportive care programs that promote healthy aging and social connection. Ensure programs are accessible, inclusive, and responsive to the diverse needs of seniors, including those with complex health and social challenges. Integrate evidence-informed approaches to enhance client engagement and wellbeing. Stakeholder & Community Engagement Collaborate with internal teams to strengthen cross-program integration and service delivery. Build and maintain relationships system partners, including Ontario Health, Ontario Health @Home, Ontario Health Teams, healthcare providers, primary care teams, community agencies, and other referral sources. Represent CANES in community forums and initiatives related to seniors' care and wellness. Compliance & Risk Management Ensure all programs adhere to applicable healthcare standards, regulations, and organizational policies and procedures. Monitor and address risks related to client care, safety, and service delivery. Escalate program and operational risks to Sr. Leadership where appropriate Support accreditation and quality assurance processes as required. QUALIFICATIONS: Post-secondary school degree in a health-related discipline Registered Nurse (RN) designation strongly preferred; consideration will be given to candidates with relevant regulated health professional credentials in good standing Minimum 5 of experience in a management position within the healthcare sector Experience managing and evaluating performance of interdisciplinary teams, including clinical and non-clinical staff. Strong understanding of the needs of seniors, including those with complex health, cognitive, and social challenges. Experience with data analytics and reporting tools Knowledge of Ontario’s home and community care sector, including relevant legislation and best practices. Experience with Quality framework Experience in developing and implementing quality initiatives Knowledge of the OH&S Act & Regulations Proficient in MS Office suite products with strong computer skills Experience and knowledge of gerontology Excellent oral communication, organization and writing skills Clear Police Reference Check with Vulnerable Sector Screening Valid class G driver’s license and access to reliable vehicle Strong problem-solving, teamwork and interpersonal skills Experience working in a public health setting Experience with client relations, including liaising with internal and external teams This job description outlines the principal duties of the role and is not exhaustive.
