The Opportunity: The Community Health Nurse [CHN] provides community health promotion services that promote the prevention of disease, maintenance of health and the overall health and well-being of individuals, families and the community. The Community Health Nurse initiates treatment including health care management and therapeutic interventions in accordance with nursing statutory and regulatory standards, limits, and conditions, and employer policies and procedures. Delivers patient/client care in accordance with established standards, policies, procedures and the goals, objectives, guidelines and directions of Carrier Sekani Family Services by performing duties such as: Performing nursing skills, such as delegated medical functions and crisis intervention, such as emergency assessment and care including arranging for escort and evacuation for further intervention as required, point of care testing, specimen acquisition- including venipuncture, and arranging referral where necessary; Assessing, planning, implementing, documenting and evaluating the care needs and goals for individual patients/clients and their caregivers/families; Visit www.practicedifferently.ca and click on ‘View Opportunities’ to view the full job description for this career opportunity. Wage Range: The target hiring range for this position is $45.9057 to $59.4491 per hour. Candidates will typically be hired, transferred or promoted between the minimum and midpoint of the wage range. Placement within the wage range is dependent on the applicants’ role specific knowledge, skills, abilities, related experience and in consideration of factors such as market conditions and internal equity.
