CBI Home Health is looking to hire a Care Supervisor, Integrated Care Programs in Hamilton. Salary Range: The salary range for this position is $70,000 – $75,000 per year. What You Will Do: As a Care Supervisor within CBI Home Health's Integrated Care Program, you will lead a team of care providers in delivering high-quality care that supports safe and seamless transitions from hospital to home. As an RN or RPN, you will develop individualized care plans, provide clinical oversight, and supervise field staff to ensure services are delivered safely and within scope of practice. Exigences

Candidates must be registered with the College of Nurses of Ontario (CNO) as an RN or RPN. You also need at least two years of supervisory or home health service delivery experience and a valid driver's license.
