The Integrated Care Lead manages patient-centered transitions from hospital to home by creating comprehensive care plans. They also collaborate with interdisciplinary teams and stakeholders to ensure safe, sustainable care delivery.

Candidates must be a Registered Nurse in good standing with the College of Nurses of Ontario with 3-5 years of supervisory experience. Experience in both home/community care and acute care settings is required, along with a valid driver's license.
