The Licensed Practical Nurse will deliver and document nursing care in a home health setting while collaborating with an interdisciplinary team. They are responsible for reporting changes in client status and providing a supportive emotional environment for patients and their families.

Candidates must hold an active license in good standing with the CLPNA and have at least 6 months of experience as an LPN. A clear criminal record check, a negative TB test, and access to a reliable, insured vehicle are mandatory.
