Classic LifeCare is a family-owned home care agency with over 50 years of experience and Accreditation Canada accreditation. We are looking for a Client Nursing Supervisor to join our Vancouver team. Key Responsibilities * Conduct comprehensive client assessments and reassessments, developing individualized, client-centered care plans that support health goals and recovery. * Complete and maintain accurate clinical documentation, including assessments, progress notes, care plans, and discharge reports in accordance with regulatory and organizational standards. * Provide hands-on nursing care as required, including wound care management and other clinical interventions within the LPN scope of practice. * Collaborate closely with the clinical team to deliver safe, high-quality, and client-focused care. * Communicate effectively with physicians, wound care specialists, case managers, and other healthcare professionals to coordinate care and optimize client outcomes. * Partner with the scheduling team to support continuity of care, timely service delivery, and proactive communication regarding client needs or changes in services. * Build and maintain positive relationships with clients, families, and employees through regular engagement calls, visits, and ongoing support. * Address concerns promptly and professionally, utilizing strong conflict resolution skills to foster a respectful, client-centered care experience. * Ensure all clinical records and documentation are maintained accurately, confidentially, and professionally. * Participate in care conferences, team meetings, and quality improvement initiatives as required. Qualifications * Current Licensed Practical Nurse (LPN) registration in good standing with the British Columbia College of Nurses and Midwives (BCCNM). * Previous wound care experience is considered a strong asset. * Excellent communication, interpersonal, and relationship-building skills. * Demonstrated conflict resolution and problem-solving abilities. * Strong computer proficiency and experience with electronic clinical documentation systems. * Excellent organizational skills with the ability to prioritize, multitask, and work independently. * Proven ability to collaborate effectively within a multidisciplinary healthcare team. * Clear criminal record with a vulnerable sector clearance * Valid BC Driver's Licence and access to a reliable vehicle for travel throughout the Lower Mainland.
