The Case Manager will be primarily responsible for engagement, stabilization with a strong focus on transition and aftercare planning. Responsibilities will be to provide clinical consultation and guidance to the Supportive Housing program team and to ensure continued integration with other internal programs and community partners. Incorporating cultural aspects into the stabilization plan and supportive housing environment as well as case conferencing with external partners for a successful transition out of the program and with First Nations will also be a part of this role. Qualifications and Requirements: * A minimum requirement of an HBSW or related degree complimented by significant experience and demonstrated skill in related work will be considered. An MSW is preferred. * a minimum three (3) or more years related experience utilizing a variety of treatment modalities in the field of Mental Health & Addictions Services particularly relating to Anishinabek clients. * experience in supervisory or leadership role preferred * experience in a residential setting would be an asset. * Experience relating to working in a variety of Mental Health & Addictions programs in the youth/family service system. * Candidates without the specific education qualifications and where the position does not require certified qualification to practice, but who possess an appropriate combination of experience and other academic qualifications will also be considered. * Significant demonstrated experience in Adult Hours of Work: 32.5 hours per week Please apply directly at https://www.dilico.com/careers/ [https://www.dilico.com/careers/] As an Indigenous Agency Dilico gives preference to Indigenous people. The Case Manager serves as the clinical lead for a 24-hour supportive housing setting, focusing on client engagement, stabilization, and transition planning. They provide clinical guidance to the program team and coordinate with community partners and First Nations to ensure successful aftercare.

Requires a degree in Social Work (HBSW or MSW preferred) and at least three years of experience in Mental Health and Addictions services, specifically with Anishinabek clients. Experience in leadership roles and residential settings is highly desired.
