Position Vacancy: Home Dialysis Assisted Program (HDAP) – Composite Position Home Hemodialysis and Peritoneal Dialysis Position: Registered Practical Nurse Position Type: Full Time Location: WRHN @ Midtown (formerly Grand River Hospital, KW Campus) Posting Period: August 4 2026 - August 9 2026 Position Summary The Registered Practical Home Dialysis Nurse is engaged in caring for patients in the Home Dialysis Assisted Program (HDAP). The successful applicant will work in collaboration with the Home Hemodialysis (home unit) and Peritoneal Dialysis Nursing teams, Nephrologists and the Allied Health team to support the dialysis of patients in their home setting. The successful candidate will incorporate the attitude, values, knowledge, skills and professional nursing practice required to support the clinical needs in the patient’s home setting. Position Responsibilities Current registration (in good standing) with College of Nurses of Ontario Applicants with current dialysis experience in acute and chronic settings will be given first consideration – Hemodialysis and Peritoneal Dialysis. Minimum of two (2) years medical/surgical experience preferred BCLS Demonstrated successful completion of Home Hemodialysis Renal Orientation. Demonstrated assessment skills and the ability to function independently Demonstrated good interpersonal and communication skills with an ability to work with a multidisciplinary team, including other health team members, patients and family members. Demonstrated ability to work autonomously and within a team environment. Responsible, energetic individual with above average ability to prioritize and organize in the home setting. Demonstrated positive relationships with other health care professionals and able to contribute to the development of an environment conducive to excellent patient care Ability to respond to emergency situations by rapidly identifying priorities and complex patient needs Ability to prioritize and complete assignments in a reasonable length of time Demonstrated evidence of recent continuing education Demonstrated understanding of and commitment to Grand River’s comprehensive safety programs and practices, including staff, patient, and environmental safety, and participation in regular in-service education in this area Must have own reliable vehicle and valid driver’s license for home visits. The Registered Practical Nurse provides care for patients in the Home Dialysis Assisted Program, collaborating with nephrologists and allied health teams. They support dialysis treatments within the patient's home setting to improve community-driven health care.

Candidates must be registered with the College of Nurses of Ontario and possess a valid driver's license with a reliable vehicle. A minimum of two years of medical/surgical experience and dialysis expertise in acute or chronic settings is preferred.
