Location: Goffstown, NH Pay: $1,868 / week Shift: Evening Shift Job Type: Full-Time, Contract (Travel) Position Overview We are seeking an experienced and adaptable Travel LPN (Long-Term Care) for an evening shift position in Goffstown, NH. In this role, you will provide ongoing chronic disease management, daily nursing care, and routine assessments for residents in a long-term care setting. We are looking for a nurse who emphasizes person-centered care, patient safety, and seamless coordination with a multidisciplinary team to maintain our residents' quality of life. Key Responsibilities * Monitor vital signs, administer medications, and document patient responses accurately. * Perform wound care, dressing changes, and routine clinical procedures per individual care plans. * Assess changes in resident conditions and promptly communicate updates to RNs, physicians, and family members. * Assist with activities of daily living (ADLs) and rehabilitation activities as directed. * Maintain accurate medical records in the Electronic Health Record (EHR) system while strictly adhering to safety protocols. Qualifications & Requirements * Education: Graduate of an approved practical nursing program. * Licensure: Current LPN license in the state of New Hampshire (or an active Compact/NLC license). * Certifications: Current CPR / BLS certification. * Experience: 1+ years of experience in long-term care or skilled nursing facilities. * Skills: Strong communication skills with families and team members, adaptability, and proficiency in EHR documentation and care planning. Preferred Qualifications * Hands-on experience with wound care management or palliative care principles.
