Location: Toronto & Greater Toronto Area Role Type: Part-Time/Full Time/Flexible Hours/Contract (Open ended) About Us Brighter Days Health is a multidisciplinary rehabilitation coordination company supporting individuals recovering from motor vehicle accidents (MVA), slip and falls, post surgery recovery and individuals with assistance needs. We are currently seeking compassionate, reliable Personal Support Workers (PSWs) to provide community-based support to clients recovering from accident-related injuries. Role Overview PSWs will assist clients in their homes and communities with activities that support independence and safety as well as recovery following accidents. Many clients are experiencing physical injuries, chronic pain, mobility limitations, cognitive impairment (such as dementia) and/or caregiver burnout while caring from family members. This role is ideal for PSWs who enjoy meaningful one-on-one work and being part of a collaborative rehabilitation team. Responsibilities * Assist clients struggling with personal care tasks * Support mobility and safe transfers * Provide assistance with Activities of Daily Living (ADLs) * Support light household tasks related to recovery needs * Accompany clients to medical or rehabilitation appointments when required * Encourage independence and participation in rehabilitation goals * Document visits and services accurately * Communicate updates with the care coordination team Qualifications * PSW Certificate from a recognized program * Valid CPR/First Aid certification * Police Vulnerable Sector Check (current) * Previous community or rehabilitation experience preferred * Experience supporting clients with injuries, chronic pain, or elder care is an asset * Strong communication and documentation skills * Reliable transportation is considered an asset What We Offer * Part time, full time and Contract work opportunities * Community-based work across Toronto & the GTA * Flexible hours, Mostly days (no overnights), Weekend availability * Supportive clinical coordination team * Consistent referrals based on client needs and location * Opportunity to work alongside physiotherapists, occupational therapists, and rehabilitation professionals
