Open until filled Union: SUN Facility: Wolseley Memorial Integrated Care Centre City/Town: Wolseley Department: Chronic Resident Unit Type: Full-time regular FTE: 1 Shift Information: Days Hours of Work: 14 SHIFTS/3WEEKS/8HR Relief: No Float: No Field Hours: No Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: No Job Description: Reporting to the Facility Manager, the Facility Care Coordinator is responsible for ensuring a high standard of care and service is provided to patients and residents at Wolseley Memorial Integrated Care Center (WMICC). WMICC is comprised of 78 beds in Long Term Care, 12 Bed Acute Care and OPD services. As an experienced practitioner, the facility care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of client care, and support and teaching to the client care teams (including clients and their families) within a collaborative environment. Human Resources Exemption: No Experience Minimum of 3 years clinical nursing experience Recent acute and long term care experience required. Reporting to the Facility Manager, the Facility Care Coordinator ensures a high standard of care and service for patients and residents across Long Term Care, Acute Care, and OPD services at WMICC. The coordinator acts in a leadership role, providing specialized clinical expertise, coordinating client care, and offering support and teaching to care teams and families.

Candidates must possess a minimum of 3 years of clinical nursing experience, including recent experience in both acute and long-term care settings, and hold a Bachelor of Science in Nursing (BScN). Supervisory experience and clinical leadership experience are considered assets, along with a Basic Cardiac Life Support Certificate (BCLS).
