Open until filled Union: SUN Facility: Wolseley Memorial Integrated Care Centre City/Town: Wolseley Department: Chronic Resident Unit Type: Full-time regular FTE: 1 Shift Information: Days Hours of Work: 14 SHIFTS/3WEEKS/8HR Relief: No Float: No Field Hours: No Salary or Pay Band: Pay Band Nurse B $42.040 to $52.440 Travel Required: No Job Description: Reporting to the Facility Manager, the Facility Care Coordinator is responsible for ensuring a high standard of care and service is provided to patients and residents at Wolseley Memorial Integrated Care Center (WMICC). WMICC is comprised of 78 beds in Long Term Care, 12 Bed Acute Care and OPD services. As an experienced practitioner, the facility care coordinator acts in a leadership role to provide specialized clinical expertise, coordination of client care, and support and teaching to the client care teams (including clients and their families) within a collaborative environment. Human Resources Exemption: No Experience Minimum of 3 years clinical nursing experience Recent acute and long term care experience required. Supervisory experience is an asset Minimum of 3 years clinical nursing experience Experience in acute care Education Bachelor of Science in Nursing (BScN) Other Education and Training Basic Cardiac Life Support Certificate (BCLS) Experience in a clinical leadership role an asset Competencies Decision making skills Conflict resolution skills Communication skills to provide/obtain verbal and/or written opinions/advice Critical thinking skills Leadership skills Problem solving skills Decision making skills Conflict resolution skills Problem solving skills Leadership skills Communication skills to provide/obtain verbal and/or written opinions/advice Knowledge and Abilities Ability to implement, utilize & evaluate best practice models of client care Demonstrated ability to function as a team member, motivator, and facilitator Knowledge of long-term care needs of adults and their families Ability to use various clinical/health assessment tools Ability to adapt to and implement change Ability to be self-directed Ability to implement and support a collaborative working enviroment Ability to use various clinical/health assessment tools Knowledge of Psycho-social assessment in a long term care setting Ability to work independently and as a member of a multi-disciplinary team Ability to delegate appropriately. Demonstrates ability to implement coordinated, comprehensive care. The coordinator acts in a leadership role, providing specialized clinical expertise, coordinating client care, and offering support and teaching to care teams and families.
Exigences
Candidates must possess a minimum of 3 years of clinical nursing experience, including recent experience in both acute and long-term care settings, and hold a Bachelor of Science in Nursing (BScN). Supervisory experience and clinical leadership experience are considered assets, along with a Basic Cardiac Life Support Certificate (BCLS).
