Position #: 9014068 Expected Start Date: October 02, 2026 Union: SUN Facility: Gravelbourg Primary Health Clinic City/Town: Gravelbourg Department: Primary Care Sites Type: Full-time regular FTE: 1 Shift Information: Days, Standby Number of Hours per Rotation: 28 shifts of 8 hours per 6 weeks Relief: No Float: No Hours of Work: Field Hours Salary or Pay Band: Pay Band Nurse Practitioner $52.590 to $62.910 Travel Required: Yes Job description: This Nurse Practitioner (NP) role provides autonomous, evidence-informed care across continuing care, long-term care, primary health care, acute care, urgent care, and emergency settings. The position will report to and work under the direction of a manager in one of the designated program areas. Practicing within Saskatchewan Health Authority policies, CRNS standards, and legislated NP scope, the NP assesses, diagnoses, treats, prescribes, orders and interprets diagnostic tests, performs authorized procedures, and evaluates care for clients with common acute illnesses, stable chronic conditions, urgent and emergent needs, and health promotion priorities as the most responsible provider (MRP). The NP works collaboratively with clients, families, physicians, multi-disciplinary teams, community partners, and managers to support safe, person-centred, culturally responsive, and coordinated care. Human Resources Exemption: No Multi-site: * Yes Licenses * Valid Class 5 driver's license Education * Master's degree in nursing Experience * Min. 3 years RN exp. in Emergency, Acute, LTC, Continuing Care, Primary Care Knowledge & Abilities * Knowledge in research processes and program evaluation * Ability to work autonomously with high levels of responsibility & accountability * Demonstrates skills in clinical governance, research-informed practice * Ability to prioritize and manage multiple projects simultaneously * Demonstrates capacity to plan, implement and evaluate multiple assessments * Demonstrated ability to use sound clinical judgement * Knowledge in wellness and holistic models of health care * Proven conflict resolution skills * Knowledgeable a trauma informed approach to patient and family care * Ability to apply evidence-based decision making * Ability to practice autonomously within NP scope, legislation, CRNS standards * Adaptability for competing/changing patient priorities, situations, environment * Knowledge of social determinants/resource evaluation for assessment/mngment plan * Communicates through therapeutic relationships, client and family education * Knowledge in chronic disease prevention and management * Knowledge of the diversity of cultures, values and traditions * Able to practice autonomously within SHA policies, clinical practice guidelines * Ability to lead, mentor, and support staff in a collaborative team environment * Knowledge in disease and injury prevention, health protection and harm reduction * Knowledge of diverse, culturally safe and representative workforce * Respects Indigenous health priorities, community context, client values * Demonstrates leadership abilities * Ability to integrate clinical knowledge and experience into clinical practice * Working knowledge in health promotion * Integrates clinical knowledge/experience to support electronic documentation * Knowledge in significance of quality improvement * Demonstrates effective verbal and written communication skills. * Ability to take initiative * Demonstrates skills in safety initiatives and service innovation Other Education & Training * Pediatric Advanced Life Support Certificate (PALS) * Canadian Triage and Acuity Scale (CTAS) training * Advanced Cardiac Life Support Certificate (ACLS) * Basic Cardiac Life Support Level C Certificate (BCLS) Other Information * Standby may be required. * Gravelbourg is located approximately 116 km southwest of Moose Jaw * For more information on the town of Gravelbourg please visit https://www.gravelbourg.ca/ * Travel required * Registered/eligible for registration with the CRNS as RN (NP) * May be eligible for Recruitment and/or Retention Incentives * Multisite position serving Primary Health Care The Saskatchewan Health Authority (SHA) is the largest employer in Saskatchewan, employing more than 45,000 staff in a dynamic healthcare environment.
