Job Purpose As a member of the interdisciplinary team, the Community-based Licensed Practical Nurse (LPN) delivers safe, person and population-centered care across lifespan within the scope of practice. The role supports health promotion, illness/injury prevention, and care management in both clinical and community settings, in collaboration with individuals, families, and community partners. Essential Qualifications Active registration with ANBLPN and be in good standing Current BCLS certification Minimum 2 years recent and direct experience (within last 5 years) in community-based nursing or a related area such as Addiction & Mental Health, Public Health, Harm Reduction, or Primary Health is required. A combination of relevant work experience and related education training will be considered. Commitment to complete area-specific training and certifications (i.e. Mental Health Interventions, Public Health Modules, Chronic Disease Management, immunization practices) as outlined during orientation and onboarding within a defined timeframe. Possess a valid driver's license and transportation, with the ability and willingness to travel within the region to support patient care, attend client appointment and participate in community-based activities as required. A valid passport may be required for staff hired to support Campobello Island within Area 2 Good work and attendance record Ability to perform the duties of the position Key Responsibilities Patient Care: Prioritizes and delivers care to patients within a collaborative setting, seeking clinical guidance as required. Conducts timely and accurate patient assessments, responds effectively to emergent situations, and prioritizes care to ensure patient safety. Establishes therapeutic relationships with patients while adhering to professional boundaries. Provides holistic patient care and therapeutic clinical care, promotes wellness and disease prevention, health education and self-management support for individuals across the lifespan. Participates in the development, monitoring of the plan of care, including maintaining document standards, in collaboration with the health care team, patients, and family members. Supervises and facilitates client activities to support health promotion, education, and self-management, including but not limited to mental health, chronic disease and wellness programs. Leads or supports group session to promote health knowledge, self-management and wellness across diverse populations and settings. Participates in the orientation of new staff and provides ongoing mentorship.
