We are hiring a permanent, part-time (0.50 FTE) Social Worker Team Leader, Primary Care Network. Days of work: Tuesday, Thursday and half day Friday Shift times: 08:30 to 16:30 What We Offer We provide a comprehensive, competitive, and people focused employment package, emphasizing wellness, growth, work–life balance, and long-term stability. About This Position This regional role supports primary care staff across the entire Kootenay Boundary area and requires regular travel. A valid driver's licence and access to a reliable vehicle are required. Depending on operational needs, a home office may be required. Please note that hours and days of work, as well as work location, are subject to change. In this highly autonomous position within the Primary Care Network, you provide day-to-day leadership, mentorship, clinical support, and problem-solving to help teams deliver exceptional care, while also stepping in to provide direct client care when needed. Working with colleagues, physicians, community partners, and service providers, you help create seamless, client-centered experiences across the continuum of primary and community care services. You build strong relationships with local agencies and partners, fostering collaboration that enhances care, strengthens service delivery, and supports community needs. You lead the planning, coordination, implementation, and evaluation of clinical practices, helping ensure high-quality outcomes for clients and continuous improvement across services. As a trusted leader, you support staff development, champion best practices, and contribute to the ongoing success of integrated, team-based care throughout the region.
