Part-Time (22.5hrs/week) Day shifts + 1:3 weekends ( 1 year contract)

We are seeking a compassionate and reliable **Registered Practical Nurse (RPN)**to join our community visiting team. In this role you will provide high-quality, patient-centered care in clients homes. including medication administration, wound care, health assessments, and chronic disease management. You will work collaboratively with an interdisciplinary team to support clients' independence, safety and overall well-being.

The ideal candidate is organized, adaptable, and confident working indepently, with strong communication skills and a genuine passion for community-based care.

Acclaim Health announces the availability of one (1) Temporary Established Part Time Registered Practical Nurse position (equivalent to 45 hours per pay period) to work in the Halton area with a focus on the Burlington Community for a period of one (1) year. This position requires the individual to be available to work 3 shifts per week and 1:3 weekends in addition to the statutory holiday rotation.

Pay Per Visit Rate: $23.41 - $25.23 per visit

PRIMARY FUNCTIONS:

  • Provide nursing care within the Community and Clinics.
  • Organizes the day, set priorities based on continuity of care, the type of care to be given and the client’s/family’s health needs.
  • Develops, implements, evaluates, and modifies the plan of care outlining the strategies, orders and actions to achieve expected/desired outcomes.
  • Assesses the health status of the client.
  • Administers IV therapy including central line and peripheral line maintenance (IV and pump skill, CVAD skill)
  • Administers medications topically through natural body orifices, through tubes and by injections.
  • Handles wound care, acute and chronic, for certain types and stages of wounds.
  • Catheter care and maintenance skills
  • Responsible for ensuring that the lap top is synchronized with the network at a minimum of twice a day, at the beginning and end of your shift, in order to receive information on and update client records.
  • Check voicemails and fax mail on days scheduled to work.
  • Attends team/POD meetings as required.

POSITION REQUIREMENTS:

  • Minimum 1 to 2 years clinical practice in a community environment preferred
  • Current registration with the CNO
  • Current CPR certificate
  • Excellent interpersonal, communication and teaching skills
  • Palliative Experience is preferred
  • Ability to administer medications
  • Demonstrated initiative and commitment to professional development and skill enhancement
  • Valid driver’s license and access to a vehicle
