Job Summary

The Possibilities by Baycrest™ is Baycrest’s unique evidence-based memory care model. The successful candidate will support the Possibilities by Baycrest™ model through embodying the philosophy of care and principles, while bringing the key commitments under the model to life. Reporting to the Clinical Manager, the Registered Practical Nurse will execute individualized care plans and Possibilities Plans for the residents living in the new Terraces Memory Residence. The RPN works with each resident and family to achieve personal, psychological, physical and recreational goals within the Possibilities by Baycrest™ framework. The ideal candidate will be well rounded with residential, geriatric, customer service and clinical care experiences working with older adults with dementia and responsive behaviours.

Key Responsibilities

  • Community development and education, and individual counselling

  • Provides emergency and urgent care to clients as needed

  • Cares for minor episodic illness

  • Monitors and educates clients with stabilized chronic and long-term illness

Qualifications

  • Current registration, in good standing, from the College of Nurses of Ontario (‘CNO’) is required

  • Current Basic Cardiac Life Support (‘BCLS’) certificate, Level C - Automated External Device (‘AED’) is required

  • Minimum two (2) years’ experience in gerontological nursing is required

  • Minimum five (5) years’ experience in both, Primary Care Community Nursing and acute care is required

  • Demonstrated expertise in clinical assessment of seniors with complex medical, cognitive and social needs

  • Excellent interpersonal, communication and problem solving skills

  • Exceptional client service skills

  • Basic computer skills – Meditech, Microsoft Word and Excel preferred
