About the role
Full-Time Permanent Salaried (37.5 Hours Per Week)
Responsibilities: The Outreach Coordinator is responsible to oversee day-to-day programming. They support staff in the implementation of client caseloads, as well as program delivery, procedures, and staff roles & responsibilities.
Qualifications:
- Minimum 3 to 5 years practical work experience in human services ,supported housing, or with persons with disabilities/addictions.
- Knowledge and understanding of Indigenous culture/vulnerable peoples/homelessness.
- Maintain professional boundaries in and out of the workplace as outlined in the YWCA Code of Conduct and Ethics Agreement.
- Valid Class 5 Saskatchewan Driver’s Licence with clean abstract.
- Reliable Vehicle.
- Excellent communication skills (written and verbal).
- Proficient in Microsoft Office applications (Word, Excel, Power Point, Outlook).
- Must pass Criminal Record Check and Vulnerable Sector Check.
- Must be a positive role model of life and social skills for client.
- Ability to work independently.
- Good time management skills.
- Must be able to work flexible hours when needed.

Basic Duties:
- Maintain written and computerized records, compile reports and complete other program documents (including case notes, statistics, letters, etc.).
- Represent Homeward Bound Windsor in meetings as required.
- Uphold and maintain policies and procedures for Homeward Bound Windsor.
- Case plan development, tracking and execution.
- Provide a monthly report & annual report to management, and monthly calendar of events for clients.
- Set long and short-term goals with a timeline for completion.
- Ensure all incidents are properly documented and reported to manager.
- Provide transportation to client appointments as needed.
- Occasional work in non-traditional hours as assigned by management.
- Collaborate with Outreach Coordinators in other YWCA programs including Oakdale, Kindred Spirit, and Our House Supportive Recovery Centre.
Program Specific Duties:
- Develop and coordinate a plan for individuals to transition into their new housing.
- Establish positive rapport with landlords in the community.
- Assist in the implementation of support plans and case-management.
- Establish the types of supports needed for individuals that have transitioned, ex: hours, areas to work on, life skills needed.
- Assist in setting up community supports once transitioned, such as educational applications, counselling, parenting programs etc.
- Review communications board, emails and log books when coming on shift;
- Work closely with the Homeward Bound Outreach Coordinator for house checks, cover off, support transitioning clients, and programming.
- Include outreach clients in building programming.
- Provide leisure-based programming with outreach clients on a weekly basis.
- Develop and participate in Homeward Bound community-wide activities.
- Support staff members at Homeward Bound Windsor as needed.
- At all times, be aware of the Homeward Bound Windsor environment and do your part to ensure it is safe, clean and welcoming.
- Meet hourly support requirements for each client on caseload.
- Oversee the cleanliness of the home and ensure maintenance is done in a timely manner.
- Other duties and responsibilities as deemed necessary by management, to ensure the effective operation of Homeward Bound
