The role of the Community Memory and Support Services Coordinator is to support the delivery of day to day resident services, in particular for Memory Care residents and Assisted Living residents. These services are in some cased tailored to specifically meet the needs of individuals and in other cases will be delivered to a group of residents. The role of the Community Memory and Support Services Coordinator will be pivotal in ensuring that all front-line teams are delivering services in alignment with the Riverstone standard, and all resident and family expectations.

Specifically, the Community Memory and Support Services Coordinator is responsible for:

• Provide on-the-floor coaching and training in the delivery and development of care approaches, including behavioral support strategies, programs and services to residents, providing corrective recommendations as needed.
• Audit resident care and needs on the floor and ensure the care needs are reflected in the care plan.
• Review Team routines with Director of Care and the Activities Manager, regularly, and auditing workflow in the homes. Ensure these routines are reflective off residents current needs.
• Serve as a liaison for residents, care teams and Director of Care to ensure work flow and care streams are up to date.
• Maintain consistent follow up with Regional Director of Care to identify risks and work with community management on action plans for continuous improvement.
• Maintain records of care audits, workflow audits, Onsite team training/coaching, and review documentation for compliance with RHRA related to direct care
• Support the communities and management teams to oversee compliance and provide feedback on, with the Incontinence Management Program, GPA Training, Behaviour Management, Care Plan updates, Dietary Plans, Pericare and skin integrity and support training, and Behavior Management Strategies Etc.
• Troubleshoot issues on the floor with the teams to promote a safe and supportive environment.
• Oversee the quality and consistency of Memory Care programming to ensure activities are meaningful, ability-based, and aligned with resident needs
• Support communication with families regarding resident care, behavioral changes, and engagement approaches, in collaboration with the leadership team
• Maintain a consistent presence on the Memory Care floor to observe practices, provide real-time coaching, and model expected standards of care and engagement
• Provide documented feedback to leadership to support staff performance management, development, and recognition

Knowledge Skills and Abilities:

• Three to Five Years of experience working in Retirement, Care Programming, or other related experiences.
• Three to Five Years of experience specifically working in dementia care, and assisted resident programming.
• Post Secondary Education in a relevant area of Healthcare, Gerontology, Social Services, or other related field of study.
• Detailed understanding of the changing needs in seniors care and aging as it pertains to dementia care and assisted living residents.
• Understanding of, and ability to apply constructively, program and leadership best practices, legislative regulations, and company policies, with service improvement at the forefront of your decision making.
• Ability to supervise multiple teams, with effective communications skill both written and in person.
• Ability to stay organized and prioritise efficiently
• Support Teams and assist in the management and resolution of conflict.
• Experience with YARDI, is a strong asset.
