Job Description Title: Certified Diabetes Educator Reports to: Manager, Primary Care Position Type: Permanent Full-Time (35 hours per week) Salary: $66,757.60 - $78,533.00 annually Vacancy Status: Existing Vacancy Location: Napanee Position Summary This specialized position requires a Certified Diabetes Educator certification, or previous experience with Diabetes Education and willingness to complete certification. The Registered Nurse (RN) or Registered Dietician (RD), Certified Diabetes Educator (CDE), provides community-based care in the prevention and management of diabetes. They participate in the development, implementation, monitoring and evaluation of programs and services for the community, individuals and families that are affected by diabetes. The RN/RD CDE works closely with the Registered Dietitian CDE and others to achieve a successful regional Diabetes Education Program. The RN/RD CDE acts as an ambassador for the Napanee Area Community Health Centre in the community and promotes positive partnerships with other providers to maximize the capacity for diabetes interventions in South East Ontario. Key Responsibilities Service Delivery Delivers, or assists with the delivery of prediabetes, diabetes and prevention education programs throughout Napanee and Area, in any community setting where the demand exists. Involved in the delivery of insulin initiation and management. Collaborates with partner agencies/hospitals in any community setting where demands exist in delivery of prediabetes, diabetes and prevention education groups. Educates, counsels and provides information to clients and/or programs within scope of practice. Ensures record keeping is up-to-date. Assists in reviewing resource packages available for clients and adapts as necessary. Identifies the need for translation of materials to meet the needs of the client base. Aware and open to the implementation of alternate forms of treatment available to assess and treat clients. Acts as a health advocate for clients. Provides home visiting services to clients as needed. Work as a part of an interdisciplinary team, including close collaboration (e.g., shared appointments) with the diabetes Nurse to offer optimal care for clients to ensure clients receive comprehensive, competent and appropriate health care. Screen, assess and provide appropriate referrals for clients, including those without appointments. Provide client screening, assessments, and health education within scope of practice and in keeping with the Diabetes Canada Clinical Practice Guidelines. Initiate, plan and facilitate groups to support clients achieve optimal health. Assist reception staff and other practitioners in screening client phone calls as needed. Provide on-going follow-up treatment services including communication through various means with clients, other staff, and external providers as appropriate. Collaborate with Primary Care Providers to obtain pertinent information (eg. Bloodwork) to provide individualized care for patients. Outreach Facilitates access to diabetes information, education & support. Participates in diabetes screening events through collaboration with other agencies. Participates in media events where required. Organizes events and meetings with community-based health providers to keep them informed of program services and updates. Responsible for organizing outreach services for clients within the community to offer diabetes care and prevention. Administrative Complete appropriate and up to date chart documentation in the Electronic Health Records for both individual and group encounters and support Quality Improvement initiatives by documenting symptoms and conditions and referrals and recommendations for ongoing treatment. Receive and track referrals from staff and self-referrals from clients; maintain schedule and wait list. Monitor and adjust workflow to ensure program targets are met. Participate in chart reviews and case conferences. Identify areas where development of protocols and procedures are needed to improve client services or to promote more effective staff functioning. Review and update Medical Directives as needed that pertain to Diabetes Education Program. Participate in chart audits as directed by organization and clinical team. Organizational Responsibilities Comply with all relevant legislation and KCHC policies, including privacy laws Commits to acquiring an understanding of the importance of trauma responsiveness and the impact of Adverse Childhood Experiences (ACEs) Commits to demonstrating an ongoing commitment to Equity, Diversity, Inclusion, Indigenization and Accessibility (EDIIA) by representing the diverse nature of our communities, promoting and practicing inclusion Supports consistent application and development of KCHC policies and procedures Supports KCHC’s student and volunteer placement programs Promotes awareness of and participation in KCHC activities Demonstrated commitment to continuous learning and quality improvement On occasion, perform other temporary duties as required Basic Education and Experience Requirements Undergraduate degree in foods and nutrition from a recognized university Current registration as a Registered Dietitian with the College of Dietitians of Ontario Demonstrated experience working with people who have diabetes A minimum of 3 years of relevant community nutrition experience, preferably working collaboratively with other health care professionals to serve clients with complex needs Knowledge, Skills and Abilities Certified Diabetes Educators (CDE) certification or in progress an asset Experience working in rural areas, with Indigenous health issues, and seniors are assets Experience in program development, implementation, monitoring and evaluation an asset Proficiency in the use of computers and various software applications Awareness of, and ability to support, provincial and federal privacy regulations Excellent oral and written communication skills Excellent organizational skills and above average attention to detail Excellent communication and interpersonal skills, and a commitment to being a “team player” Other Requirements Valid Driver’s License, valid insurance coverage, and access to vehicle is required as this position may be required to travel to rural areas Current and satisfactory Criminal and Vulnerable Persons Check French or Indigenous language is an asset As a registered professional, to abide by and be accountable to the ethics and standards set out by the relevant regulatory body of the profession. Exigences

Candidates must hold an undergraduate degree in foods and nutrition and be registered as a Registered Dietitian with the College of Dietitians of Ontario. A minimum of 3 years of relevant community nutrition experience and a valid driver's license are required.
