Social Services Coordinated Entry Outreach Facilitator

Our Coordinated Entry program in Red Deer is seeking a Permanent Full Time Coordinated Entry Outreach Facilitator The Coordinated Entry program is for youths between 15 and 24 who have been homeless for three months or have a history of episodic homelessness that spans a significant portion of the last few years. Referrals are accepted based on: youth without a permanent place of residence; living on the street, in shelters or places not intended or suitable for permanent residency; “couch surfing”, or living off others for short periods of time. An interview with the youth/family being served is also part of the process. Prioritization includes reviewing all available information, discussions with referral sources and information obtained for admission and screening reports Minimum Qualifications and Requirements: * Diploma at minimum, preferred degree directly related to Human Services * Required 1 year of frontline human services experience * Proof of Membership in Professional Association (when applicable) * Knowledge and understanding of SPDAT considered an asset * Basic First Aid Level C with adult, child, and infant CPR &AED * Suicide Intervention Training & De-Escalation Training * Intervention Record & Criminal Record Checks with the vulnerable sector * Class 5 drivers license, clean drivers abstract and reliable vehicle for client transportation required. Responsibilities include but are not limited to: * Develop relationships with homeless youth. * Work in partnership with Adult Coordinated Entry * Support the warm transfer of youth from intake to housing * Access appropriate community support for youth * Work with external organizations to facilitate transitions and wraparound services * Work to promote the program within the community Wage Range: $23.46 - $26.24 per hour. Plus, Travel Reimbursement at $0.57/KM. Requires a diploma or degree in Human Services and at least one year of frontline experience. Must possess a Class 5 driver's license, a reliable vehicle, and certifications in First Aid, CPR, and Suicide Intervention.
