VACANCY NUMBER: 43893NC JOB TITLE: GERIATRIC ASSESSMENT NURSE, TFT STATUS: TEMPORARY FULL-TIME DEPARTMENT: GERIATRIC ASSESSMENT - GERIATRICS LOCATION: MULTI-SITE HOURS OF WORK: 75 HOURS PER PAY PERIOD, SHIFTS: 0800-1600, MONDAY - FRIDAY POSTING DATE: 27 JULY 2026 CLOSING DATE: 02 AUGUST 2026 UNION: ONTARIO NURSES' ASSOCIATION WAGE RANGE: $43.970 to $61.740 per hour POSITION OBJECTIVE: The Geriatric Assessment Nurse performs comprehensive geriatric assessments, including but not limited to: cognitive testing, physical assessments, dementia/delirium/depression screening, in order to develop treatment recommendations and/or plans for inpatients and outpatients. The nurse performs case management which consists of medication management and titration, laboratory and diagnostic testing follow-up, design and ongoing evaluation of behaviors and safety plans of care as well as timely communication with the interprofessional team to support the plan of care and patient flow priorities. Providing family education, comprehensive case management, working with community partners, completing written reports, and making recommendations for referrals are essential to the role. In addition, patient education and knowledge of community resources, and strategies to prevent avoidable admissions is required as well as the participation in strategies to enhance standardization processes and procedures across sites. Performance in the role is congruent with the College of Nurses of Ontario (CNO) Standards of Practice and policies and procedures of Niagara Health. QUALIFICATIONS: o Current registration as a Registered Nurse or Registered Nurse Extended Class in good standing with the College of Nurses of Ontario o Canadian Nurses Association Gerontology Certification o Current Gentle Persuasive Approaches Certification o Minimum five (5) years recent RN experience in inpatient, outpatient, and/or LTC geriatric assessment with a strong theory based conceptual background o Experience with Comprehensive Geriatric Assessments o Completion of an approved Geriatric Certification program o PIECES training o Recent experience with triage and the ability to prioritize referrals o Superior inter-personal and leaderships skills including critical thinking, responsiveness, conflict resolution, visionary thinking, organizational skills, effective customer service, decision-making skills and problem solving o Current BCLS certification o Current knowledge of cognitive impairments, geriatric syndromes, disease, pathophysiology, and psychosocial treatment modalities o Recent experience with case management, knowledge transfer, and coaching for patients and families o Experience with facilitating group family meetings resulting in outcome resolution with effective and efficient use of health care resources o Excellent cognitive and psychological assessment skills using standardized tools o Demonstrated assessment skills and ability to develop plans of care for complex issues with older adults o Excellent communication skills with the ability to relay complex information effectively to patients, families and health care professionals o Demonstrated knowledge, understanding, and application of the Canadian Gerontological Nursing Standards of Practice and Competencies o Current knowledge of community agencies, long term care, and other health resources within the community o Working knowledge of applicable Provincial legislation o Recent experience with patient transitions through the inpatient and outpatient services and the health care continuum o Integrates reflection, critical thinking, education and relevant practical experience in problem solving o Demonstrated initiative to participate in Continuing Education, Program Activities, Committees and In-services o Demonstrated leadership skills (leadership courses an asset) o Demonstrated ability to triage priority referrals o Demonstrated knowledge and application of CNO Standards of Practice o Knowledge of Risk Management policies and procedures o Demonstrates initiative to participate in and promote continuous quality improvement o Excellent working knowledge of computers including proficiency working in Microsoft Office, Excel, etc. o Superior human relations skills with demonstrated skills in conflict resolution and innovative problem solving o Strong analytical, problem solving, organization, time management, and communication skills which allow for independent practice and the ability to work as a member of an interdisciplinary team o Consistent and regular attendance required Questions, quoting the job posting number, may be directed to hrrecruit@niagarahealth.on.ca How to Apply: If you are interested in this position, please visit our careers page by 11:59pm August 2, 2026 and submit an application through eRecruit. Exigences
Requires registration as an RN or RNEC with the CNO and a minimum of five years of recent geriatric assessment experience. Must hold certifications in Gerontology, Gentle Persuasive Approaches, and BCLS.
