As a member of the collaborative care team, the Care Team Assistant is responsible for supporting patients' activities of daily living and instrumental activities of daily living. The Care Team Assistant also reinforces patient and family teaching according to established guidelines and the plan of care. About You We would love to hear from you if you have the following: Continuing Care Assistant Program Provincial Certification with Department of Health and Wellness Candidates are required to submit their certification and be registered and active with the CCA Registry.
