POSITION SUMMARY: The Case Worker in Ontario Structured Psychotherapy (OSP) reports to the Clinical Manager of OSP. The successful candidate will be part of a team responsible for the coordination of OSP including: triaging of all referrals received to OSP; communication with referral sources to ensure complete and appropriate information is received; presentation of complete referrals to intake clinician responsible for acceptance to the program. KEY DUTIES AND RESPONSIBILITIES: * Ability to make support referrals * Triaging referrals received by Ontario Shores * Facilitate admission to Ontario Shores for Ontario Structured Psychotherapy * Facilitate referrals for community support/intervention. * Reflecting in depth knowledge of community resources within the OSP Catchment area * Communication with referral sources to ensure complete and appropriate information is received * Provide information and system navigation to individuals, families and other care providers. * Responsible to recognize and report patient, staff, and environment safety issues, identify and implement preventative and corrective actions, within the scope of this role. * Implements and models the Recovery philosophy in all aspects of care as a Recovery champion * Variable hours may be required, due to patient demands QUALIFICATIONS: * Registration as a Healthcare Provider, Undergraduate Degree, or possession of a current certificate of competence as a Registered Practical Nurse if applicable. * Current CPR-HCP is required. * Current driver’s license * Ability to perform the physical demands of the position. KNOWLEDGE: * Sound knowledge of community resources in mental health and addiction. * Demonstrated advanced knowledge of psychotropic medication. * Thorough knowledge and ability to administer mental status examinations, psychosocial and crisis assessments. * Demonstrated knowledge of the College Standards of Practice and commitment to maintaining reflective practice standards as described by the College. * Demonstrated knowledge of Mental Health, and relevant policies and legislation, such as the Mental Health Act and PHIPA is required. SKILLS: * Ability to work effectively individually as well as part of an interprofessional team. * Strong collaborative skills in working with community partners and health care providers * Excellent interpersonal and effective oral/written communication skills including demonstrated excellent documentation practices. * The ability to work with clients of diverse ethno racial and cultural backgrounds and proficiency in a second language would be considered an asset. YEARS OF EXPERIENCE: * 3-5 years psychiatric experience in a mental health facility or service. Candidates must hold an undergraduate degree or a Registered Practical Nurse certificate, along with 3-5 years of psychiatric experience. A current CPR-HCP certification and a valid driver's license are also required.
