Job Summary We are seeking a dedicated and compassionate Nurse Practitioner to join our healthcare team. The ideal candidate will provide high-quality patient care in our PCC patients. This role requires a strong focus on pain management, ensuring that patients receive comprehensive assessments and appropriate treatments. The Nurse Practitioner will work collaboratively with other healthcare professionals to deliver exceptional patient outcomes. Responsibilities * Perform comprehensive patient assessments and develop individualized treatment plans. * Order diagnostic tests, prescribe medications, and administer pain interventions within their scope of practice. * Work collaboratively with physicians, specialists, and other healthcare professionals to ensure seamless care for patients with acute and chronic pain conditions. * Educate patients on pain management strategies, lifestyle modifications, and preventive care. * Support mental health needs through assessment, diagnosis, and management. * Provide holistic care that considers the physical, emotional, and psychological well-being of each patient. Experience * A valid Nurse Practitioner license is required. * Active registration with the College of Nurses of Ontario (CNO). * A minimum of one year of clinical experience in primary healthcare, urgent care, or outpatient settings. * Previous expertise in pain management, chronic disease management, family medicine, or Musculoskeletal Disorders preferred * Familiarity with medication administration protocols and pain management techniques is essential. * Strong communication skills to effectively interact with patients, families, and healthcare teams. * Ability to work independently while also being a collaborative team player. * A passion for improving access to specialized healthcare services.
