Mentor (Future Planning and Network Facilitator)

Who will be there when we no longer can? This is the question that led families to create PLAN more than 35 years ago. They believed the strongest safeguard for the future is a network of caring people—people who know someone well, value their gifts and remain involved as life changes. That belief continues through PLAN’s Lifetime Membership and the work of our Mentors. We are looking for an experienced facilitator and relationship-builder to join PLAN as a Mentor, helping Lifetime Members and their families strengthen personal support networks and plan thoughtfully for the future. The Role Mentors take time to know the person—not only their support needs, but their gifts, interests, relationships, hopes and the things that bring meaning to their life. They bring people together, help relationships grow, facilitate conversations about the future and support families through change. This is not a support-worker, clinical or traditional case-management position. Mentors do not take over or make decisions for people. They listen deeply, ask thoughtful questions, recognize possibilities and help people move toward the future they envision. As a Mentor, you will: Build trusted relationships with Lifetime Members and families. Help develop and sustain personal support networks. Facilitate network gatherings and future-planning conversations. Support families through important transitions. Create pathways to friendship, contribution and belonging in community. Help families navigate systems and connect with appropriate resources. Collaborate with families, networks, Community Connectors, colleagues and community partners. Contribute to a reflective and learning-focused team. About You You are curious about people and attentive to what matters to them. You can listen without rushing to fix, advise or take over. You see gifts and possibilities while remaining thoughtful about challenges. You bring: Significant experience working alongside people with disabilities and families. Strong relationship-building and facilitation skills. Experience supporting people through change, complexity or important life transitions. An understanding of person-centred, asset-based and relationship-centred practice. Maturity, sound judgment and respect for professional boundaries. Strong communication, organization and follow-through. Your experience may come from community development, social work, disability, family support, facilitation, counselling, education, human services or lived experience. The Mentor builds trusted relationships with Lifetime Members and their families to develop and sustain personal support networks. They facilitate future-planning conversations and help families navigate systems to ensure community belonging and support during transitions.

Candidates must have significant experience working with people with disabilities and families, specifically in relationship-building and facilitation. A strong understanding of person-centred and asset-based practices is required, along with maturity and sound professional judgment.
