Provide specialized nursing care in the community, focusing on peritoneal dialysis, wound care, and IV management. Deliver clinical assessments and patient/family education while maintaining accurate charting.

Must be a CNO-registered RN or RPN with a current BCLS certification and a valid driver's license with a reliable vehicle. Requires a clear Vulnerable Sector Check and up-to-date TB testing.
