Case Manager – Prenatal Outreach Resource Team (PORT) Job Category: Case Manager Job Type: Full Time Job Location: Regina PORT Case Manager Sanctum Care Group is seeking dedicated and passionate individuals to join our Prenatal Outreach Resource Team (PORT). This team delivers comprehensive, wrap-around support services to expectant women in Regina who are at risk of having their baby apprehended. PORT’s goal is to enhance health, wellness, stability, and parenting readiness through collaborative, community-based engagement. Working closely with community partners, PORT Case Managers will support expectant mothers by coordinating psychosocial, mental health, addictions, and physical health services. Team members will address barriers as they arise and maintain a strong, consistent focus on the wellbeing of both the women and their unborn babies.
