POSITION SUMMARY The FASD Program Mentor will be responsible for providing a support system that will assist clients & families, by applying the Home Visitation Model of Care. Mentorship will include the development of a comprehensive service plan that includes support for family dynamics, addictions and overall wellbeing. Duties will include conducting family assessments, developing and implementing short term interventions (including drug & alcohol treatment) through effective case management to stabilize families in crisis and refer to existing community resources for longer term client & family support programs REPORTING STRUCTURE Reports to: FASD Coordinator KEY RESPONSIBILITIES Develops comprehensive service plans that meet the needs of the client/family. In conjunction with the family and client, identifies and prioritizes client/family goals and needed services that will assist with client/family stabilization (i.e., income support, treatment services, housing, food, clothing, etc.) in the form of a Service Plan. Develops and organizes Comprehensive Service Plan reviews with client/family and partnering agencies. Facilitates client/family access to essential services. Liaises with Partnering Agencies as needed and makes referral in Child Protection Services when necessary (i.e., risk assessment). Refers client/families to appropriate services within the community. Assist the Coordinator in the development and implementation of a quality improvement plan. Keep updated on drug and alcohol trends. Participates in extensive training as identified by the Coordinator. Supports and reinforces the principles, purpose and processes associated with the program. Advocate for the individual and/or family to other service areas and organizations in regard to a diagnosis and implications of an FASD diagnosis Teaches basic life skills and assist the client/families with parenting and household management. Seek stable housing options to secure a safe environment for the family and children (Includes on and off reserve agencies). Maintains efforts to access ‘hard to reach’ clients/families who have not refused services, but have not fully accepted services as well. Promotes and maintains good public relations Ensures client confidentiality in all matters related to diagnosis and file management Maintains and strengthens positive working relationships with other service providers Uses multiple approaches to establish contact with families on creative outreach. Provides mentorship and support to professionals and paraprofessionals. Ensure consent and release forms are signed for the gathering of client information Ability to conduct interviews covering extremely sensitive and confidential issues Provides supportive team atmosphere to staff and associated professionals. Assists the Coordinator with public presentations regarding the program. Establishes and nurtures relationships among partnering agencies, service providers, schools, professionals and businesses to maximize community knowledge of the program services Report to Coordinator for weekly clinical supervision. QUALIFICATIONS Education Minimum of a Certification in Social Work, Psychology, Sociology, Addictions or related Human Services field; preferred Diploma. Experience One (1) year experience in the addictions field. Exceptional computer skills Familiarity in working with medical and professional individuals/organizations Extensive knowledge in working with the First Nations community; knowledge of Blackfoot culture and/or language an asset REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES Knowledge, Skills and abilities: Knowledge of interviewing and information gathering techniques Demonstrates a high level of writing ability; able to write reports in a coherent and competent manner Ability to provide tactful, courteous and effective service with Siksika Nation clients, agencies, and relevant parties Ability to communicate effectively with internal and external clients and to meet their needs in a polite, courteous, and cooperative manner Displays a high level of initiative, effort and commitment towards completing assignments efficiently Works with minimal supervision Demonstrates responsible behavior and attention to detail Responds appropriately to supervision; listens to others and responds appropriately Follows the appropriate policy and cooperates with supervisor Expresses information to individuals or groups effectively, taking into account the audience and nature of the information

Exigences

Candidates must have at least a certification in Social Work, Psychology, Sociology, Addictions, or a related Human Services field, with a diploma preferred. One year of experience in the addictions field and extensive knowledge of working with First Nations communities are required.
