It’s the way you want to be treated. Position Summary: Coordinates the care of patients through the disease process. Acts as a clinical liaison for patients, across medical services and clinical departments. Assesses clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Evaluates and manages patient symptoms before and after treatment. Essential Duties and Responsibilities: Act as liaison for patients, families and their multidisciplinary clinical team. Assess clinical, emotional, spiritual, psychosocial, financial, and other patient needs. Facilitate resolution to patient needs and monitors follow up. Educate/reinforce education with patients and families on disease, treatments, side effects, adverse reactions, and reportable signs/symptoms based on multidisciplinary care plan. Coordinate discharge planning with the patient’s family, social workers, home nursing staff, oncologists, and personal physicians. Work with patients and family members to ensure discharge instructions are understood and followed. Develop and/or coordinate development of patient education materials, clinical pathways, and quality studies. Coordinate and/or participate in multidisciplinary consultations and/or tumor conference. Document recommendations made at multidisciplinary consultations and/or tumor conferences. Help develop a treatment plan that is understandable and feasible to the patient. Serve as clinical partner and escalation point for Intake Coordinators. Promote community education and increases prevention awareness and early-detection screening programs. May be responsible for direct patient care Perform other duties assigned Adhere to Silver Cross policies and professional standards. Promote a clean and safe environment of care, utilizing the SAFE error prevention habits. Provide the highest standard of privacy and confidentiality in matters involving patients, coworkers, and Silver Cross by abiding by the Standards of Conduct Required Qualifications: Education and Training: Registered Nurse with a current Illinois license. Graduate of an accredited School of Nursing with BSN. Oncology Certified Nurse preferred. Oncology Nursing Society Chemotherapy Provider preferred. Minimum of two years hematology/oncology clinical nursing experience with demonstrated clinical competence in patient care, teaching and management of patients with a cancer diagnosis, and knowledge about clinical aspects of oncology including protocol therapy and data management. Must have general computer skills and be adaptable to changing working situations and work assignments. Excellent interpersonal skills required, with specific ability to communicate effectively with patients, family members, and physicians. Illinois nursing license required. OCN Certification preferred. ONS Chemotherapy Provider preferred. The expected pay for this position is listed below: $35.00 - $42.50 The final pay rate offered may be more than the posted range based on several factors including but not limited to: licensure, certifications, work experience, education, knowledge, demonstrated abilities, internal equity, market data, and more.

Exigences
Candidates must be a Registered Nurse with a current Illinois license and a BSN degree. A minimum of two years of hematology/oncology clinical nursing experience is required, with OCN and ONS Chemotherapy certifications preferred.
