Position Details Job Number: JR01502 Department: Independent Home Therapies Dialysis HHD PD This position may be scheduled at any of the following sites: Brantford General Hospital Satellite Hemodialysis Unit (Brantford), Charlton Campus (Hamilton Downtown), King Campus (Stoney Creek East Hamilton), Six Nations Satellite Hemodialysis Unit (Ohsweken) Job Type: Regular, Full time Contract Duration: (If applicable) 0 months Number of Positions: 1 Union: ONA Local 75 Hourly Rate (Min / Max): $43.81 - $61.54 Schedule Work Days: Monday to Friday Time of Day: Days Shift: Hours - 10, Hours - 7.5 Shift Start: (If applicable) Reason for Posting: Posting is for a role with an existing vacancy Application Dates Opening Date: 2026-08-13 Closing Date: 2026-08-20 Job Description UNIT PROFILE: The Home Dialysis Program provides comprehensive and collaborative education, assessment, and ongoing support for patients and families/support persons who perform hemodialysis or peritoneal dialysis in the home setting. Nurses in this program promote patient independence through consistent, evidence informed teaching and clinical support whether during clinic follow up appointments, at patient’s homes or when providing dialysis training sessions. Care is provided in alignment with unit policies, provincial regulatory standards, and best practice guidelines for home based renal replacement therapy. QUALIFICATIONS: •Current registration with the College of Nurses of Ontario required •Current experience in hemodialysis or peritoneal dialysis (2 years) required •Demonstrated recent leadership experience in renal or related nursing required, as evidenced through relevant roles, education, and/or professional activities, including but not limited to: -Experience in Charge Nurse and/or Team Lead roles -Completion of additional nephrology or transplant-related education (e.g. courses/certificate, cNeph) -Active participation in, or leadership of, committees -Delivery of nephrology or transplant-related educational presentations •Current BCLS/HCP certification required •Canadian Nurses Association certification in Nephrology cNeph(C) preferred •Minimum of one (1) year of independent modalities experience preferred •Membership with RNAO or another relevant professional association preferred •Current CPI and/or GPA training preferred •Excellent attendance record is required •A resume outlining experience, further education, and any special courses is required and must accompany the application. DEMONSTRATED KNOWLEDGE, SKILLS AND ABILITIES: •Performs and documents a comprehensive health history, physical and psychosocial assessment •Ability to conduct meetings with patients and families in their home as required •Acts as a priority contact person for the patient with CKD •Demonstrates cultural competence and respect for the healthcare philosophy of patients and their families •Advanced case management, organization, time management, and coordination skills with the ability to prioritize workload to meet urgent and rapidly changing timelines. •Self-directed with strong leadership, problem solving, decision making, critical thinking, team building and interpersonal skills. •Demonstrated ability to effectively communicate and provide education to patients and families. •Demonstrated ability to practice collaboratively and communicate clearly with members of the interdisciplinary team to provide care for a range of patients/clients. •Demonstrated proficiency with computers (e.g., Microsoft Office [Word, Outlook, Excel, etc.], internet browser searches, data entry and patient information systems). •Demonstrated commitment to ongoing learning, professional development and continuous quality improvement.
