Nurse Practitioner - Primary Care Position Summary We are seeking a Nurse Practitioner (NP) to join a multidisciplinary healthcare team providing comprehensive primary care services. The successful candidate will focus on health promotion, disease prevention, patient education, and ongoing management of acute and chronic conditions. Care may be provided in clinic settings, patients' homes, and community-based environments as needed. Responsibilities Provide comprehensive primary care services to patients across the lifespan. Conduct advanced health assessments and diagnostic evaluations. Develop and implement treatment plans within scope of practice. Promote health education, disease prevention, and patient self-management. Collaborate with physicians and other healthcare professionals to deliver high-quality patient care. Maintain accurate and timely documentation within electronic medical records. Participate in community outreach and home visits as required. Qualifications Current registration as a Nurse Practitioner (Extended Class) in Ontario. Bachelor's Degree in Nursing; Master's Degree preferred. Strong clinical assessment, diagnostic reasoning, and patient management skills. Experience in primary care is preferred. Proficiency with electronic medical records and computer applications. Excellent communication, organizational, and time-management skills. Ability to work independently and collaboratively within a multidisciplinary team. Valid Ontario driver's license and ability to travel locally. Current CPIC (Criminal Record Check) required.
