We are GROWING and have new positions available to expand our Primary Care Action Teams. Thames Valley Family Health Team is looking for a Registered Practical Nurse. This position would initially be based out of 450 Central Ave. in London, Ontario with an expected transition to a Lambeth site in Fall 2026. TVFHT is one of Ontario’s largest and most dynamic family health teams, serving patients across over 21 sites in four counties. Our interdisciplinary teams including physicians, nurse practitioners, nurses, pharmacists, social workers, dietitians, and other allied professionals work together to provide comprehensive, patient-centered care. Skills & Qualifications Required Current registration in good standing with the College of Nurses of Ontario An interest in and experience facilitating and delivering both group and individual patient education Ability to provide care in accordance with full scope of practice, established medical directives and direct orders Knowledge and experience working collaboratively with community resources Knowledge of community resources to support primary care Understanding of the Chronic Disease Management and Patient Self-Management Models in Preventative Care Proficiency in the use of computer and previous experience with an Electronic Medical Record (EMR) Must be capable of establishing positive interpersonal relationships and able to work as an effective team member Must have reliable transportation to allow you to travel to other locations to complete your work if needed Hourly Rate: $28.3716 - $34.5995 To Apply: Interested candidates are invited to submit a cover letter and a detailed resume.
