Job Description: The In-Patient Medicine Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. We are dedicated to building a diverse workforce that reflects the community in which we live and serve. Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage. Required Qualifications: * Current Registration with the College of Nurses of Ontario as an R.N. * BScN Preferred * Advanced Clinical Preparation with a minimum of 5 years of sound clinical experience in general med/surg, respiratory, and/or cardiac surgical areas * Certification and/or clinical preparation in a relevant specialty a definite asset * Demonstrates initiative, creativity and innovation in clinical practice * Excellent critical thinking and decision making skills * COPD Mitchener Institute Educator Course an asset * A demonstrated commitment to patient centered care * Ability to promote change and support and mentor others through the change process * Demonstrates continuous learning and professional development * Well developed computer skills * Actively supports, and role models the purpose, core values & vision of Trillium * Strong communication skills along with the ability to establish positive relationships/partnerships with all staff, patients, families and external partners * Your leadership, facilitation, decision making, conflict resolution and clinical abilities will promote and encourage staff empowerment * Plans, organizes and co-ordinates the day to day activities of Medicine 3C * Provides clinical support to staff and clients * Identifies gaps in service and makes recommendations to address issues * Designs, implements and evaluates educational programs/strategies that are evidence based and support best practice in the management of patient care * Acts as a link with the Clinical Educator(s) and support the implementation of new educational initiatives for staff * Consults and facilitates with community agencies if required * Participates in the recruitment, retention and evaluation of staff Job Responsibilities: * Monitors quality indicators including patient satisfaction * Plans, organizes and co-ordinates the day to day activities of Medicine 3C * Provides clinical support to staff and clients * Identifies gaps in service and makes recommendations to address issues * Designs, implements and evaluates educational programs/strategies that are evidence based and support best practice in the management of patient care * Acts as a link with the Clinical Educator(s) and support the implementation of new educational initiatives for staff * Consults and facilitates with community agencies if required * Participates in the recruitment, retention and evaluation of staff To pursue this career opportunity, please visit our website: www.trilliumhealthpartners.ca [http://www.trilliumhealthpartners.ca] Internal Candidates who believe they possess the necessary qualifications and experience for this position and who have been in their current position for at least nine (9) months are encouraged to apply.
