Job Description: The In-Patient Medicine Program at THP consists of a total of more than 350 beds across the CVH & MH and is staffed by large interprofessional teams who support the care of complex medicine patients within the Mississauga-Halton LHIN. Our in-patient units offer first-rate care in an innovative environment supporting the physical, mental, emotional and spiritual needs of our patients. Reporting to a Clinical Manager, the ideal candidate will be focused on providing excellent patient centered care in a dynamic environment where he/she will be given the opportunity to deliver care with compassion, excellence and courage. RESPONSIBILITIES Clinical Practice * The Nurse Navigator possesses excellent decision making and clinical judgement skills to appropriately manage, assess and triage all the referrals and assign appropriate. * The Nurse Navigator will provide psychosocial, physical, and spiritual support, which may be complex and dynamic, utilizing the nursing process. * The Nurse Navigator will act as an expert resource to healthcare providers within the community to facilitate access to care, services and resources, and potential entry into the cancer care system. * Works in collaboration with referring physicians, the Diagnostic Assessment Program team, Diagnostic Imaging, and Laboratory departments to facilitate information sharing and flow to provide seamless care to patients and families * Provides nursing assessment and coordination of care utilizing appropriate protocols, tools and documentation processes. * Provides ongoing support and education with regards to staging and evaluation process during waiting time to surgical consult. * Provides ongoing assessment of symptoms and alters appointments/consults in collaboration with the clerical team as necessary. * Provides education and support regarding required diagnostic tests. * Integrates substantial knowledge, skill, decision making and communication techniques in assessing health needs and in planning, implementing and evaluating nursing care for the patient with a potential cancer diagnosis. * Assesses the need for additional patient/family support and facilitates access to Institution/community supportive care services, as required. * Acts as a patient advocate in obtaining timely treatment date and coordinated pre-consult and pre-surgical testing. * Individualizes the inter-professional patient care path to reflect interventions to meet identified needs while promoting safety, privacy and confidentiality, in consultation with the patient/family/significant others and the health care team. * Supports marketing and promotional activities raising awareness and increasing referrals to the Diagnostic Assessment Programs. * Assists with planning and redevelopment of established Diagnostic Assessment Programs, and the project management and planning of new Diagnostic Assessment Programs Coordination * Reviews and triages referrals to the Diagnostic Assessment Program based on pre-determined guidelines and principles. * Reviews referrals to assess and ensure completeness and coordinates obtaining any essential information for the consultation (as outlined on referral form). * Facilitates access and coordination of care and services during the patient’s diagnostic experience until the consultation visit. Education * Participates in the development and delivery of educational material to meet the learning needs of patients, family members, peers and other health professionals. * Completes health teaching for clients/families regarding each component of the diagnostic pathway. * Engages in continuous learning opportunities to maintain clinical competence and to advance own knowledge. Project Leadership * Assists in the planning and development of business case proposals and project plans to support diagnostic pathway implementation for new initiatives within the Diagnostic Assessment Program. * Facilitates the implementation of care pathways for Breast, Gynecologic, Thoracic, HPB, Prostate, CNS, and Colorectal Cancer Diagnostic Assessment Programs. * Facilitates and coordinates with regional stakeholders to gather information, accomplish specific tasks to meet goals and timelines, and provide input into regional Diagnostic Assessment Program initiatives. * Engages in creative, collaborative problem solving and planning strategies to provide information on and assist in the resolution of identified pathway delays and service gaps. * Provides input into evaluation methods and tools to measure the effectiveness of Diagnostic Assessment Programs. * Develops Diagnostic Assessment Program reports, posters, and abstracts, as required. Professional Development * Demonstrates a commitment to the philosophy and goals of the organization. * Shares expertise and provides consultation beyond the boundaries of the institution into regional, provincial, national and international communities in areas of specialization. * Demonstrates comfort and capability with presenting to large groups and with public speaking to promote the Diagnostic Assessment Program and process. * Demonstrates a commitment to life-long learning by participating in ongoing education, self-reflective practice and maintaining an expert knowledge level in caring for the patient with Breast, Thoracic, HPB, Prostate, CNS, and Colorectal cancers. * Participates in team meetings, actively participates in and contributes to multidisciplinary rounds. QUALIFICATIONS * Baccalaureate in Nursing * Registered Nurse in the province of Ontario. * Oncology Certification –CON(C) * Oncology Specialty Certificate an asset * Must agree to participate in an online Patient Navigation course (De Sousa Institute) immediately after hire and attend a one day workshop in Toronto. * Demonstrated counselling skills * Minimum of 3 years Oncology nursing experience; Surgical Oncology experience preferred * Superior interpersonal and communication skills. * Demonstrated leadership and strong organizational skills. * Embraces principles of evidence-based practice * Experience in patient education and program development * Demonstrated ability to manage programs and implement projects * Knowledge of current Mississauga Halton Central West Regional Cancer Program * Current certifications of competence from the College of Nurses of Ontario * Current membership in other professional organizations (i.e. RNAO) and the Canadian Association of Nurses in Oncology (CANO) an asset. The Nurse Navigator manages, assesses, and triages oncology referrals while providing psychosocial, physical, and spiritual support to patients. They also coordinate care pathways in collaboration with interprofessional teams and facilitate access to diagnostic services.

Candidates must hold a Baccalaureate in Nursing and be a Registered Nurse in Ontario with at least 3 years of oncology nursing experience. Oncology certification (CON(C)) is required, and candidates must be willing to complete a patient navigation course.
