The SAC Pharmacy Team Lead is an in-scope leadership role within a complex, multidisciplinary ambulatory care environment focused on advancing patient-centred care for persons living with HIV. The Team Lead works closely with the Ambulatory Pharmacy Manager, Pharmacy Clinical Practice Leader, and an interdisciplinary team of physicians, pharmacists, technicians, nurses, social workers, and staff to optimize care delivery, therapeutic outcomes, and pharmacy practice initiatives. A significant portion of the role involves direct patient care, including comprehensive medication management, identification and resolution of drug therapy problems, prescribing and monitoring therapy, and contributing to shared decision-making. The position supports pharmacy colleagues through mentorship and fosters a collaborative clinical culture while advancing quality improvement and practice innovation. Operational and dispensary oversight are key responsibilities, supporting safe and effective care delivery and accountability as Pharmacy Licensee in accordance with Alberta College of Pharmacy standards. The role supports program priorities, change initiatives, and communication with Pharmacy Services leadership. You will manage medication therapy for patients, both initially and on an ongoing basis. In addition, you will provide assessment, intervention and resolution of potential or actual drug-related problems and oversee activities related to compounding and packaging. Current, provisional or transferable registration with the Alberta College of Pharmacy (ACP) as a Pharmacist. Additional Required Qualifications: Minimum 3 years of current/recent direct patient care experience in an ambulatory and/or hospital inpatient setting. Additional Prescribing Authorization. Demonstrated experience leading practice improvement, workflow enhancement, quality improvement, or service delivery initiatives. Demonstrated experience mentoring, coaching, precepting, or supporting the development of peers and learners. Strong clinical decision-making skills, including management of complex medication regimens. Demonstrated ability to influence practice change and support interdisciplinary collaboration. Strong critical thinking, time management, and problem-solving skills, along with a compassionate, patient-focused approach. Excellent interpersonal and communication skills, with the ability to work effectively within a multidisciplinary team. Professional accountability and commitment to safe, high-quality patient care. Preferred Qualifications: Current or recent clinical experience in HIV, ID, or other complex chronic disease management. Completion of an accredited hospital residency and/or post-degree PharmD program. Additional certifications are assets. Experience in clinical research, quality improvement, change management, or service redesign initiatives. Experience implementing new clinical services, workflows, or operational processes. Experience using data to improve service delivery. Proficiency with Connect Care.

Requires a Bachelor of Science in Pharmacy and registration with the Alberta College of Pharmacy, along with at least 3 years of direct patient care experience. Additional prescribing authorization and demonstrated experience in leadership or mentoring are also required.
