As an OT in our Integrated Care Program, you’ll support safe transitions from hospital to home through functional assessments, personalized interventions, and collaboration with care teams — making a real impact where it counts. The Occupational Therapist will provide functional assessments, personalized interventions, and home modifications to help patients regain independence. They will work collaboratively within an interdisciplinary team to support safe transitions from hospital to home.

Candidates must be registered members in good standing with the College of Occupational Therapists of Ontario. A valid driver's license and insurance are required for travel within the Durham area.
