THE ROLE IN A NUTSHELL Base Salary: CAD $59,688.00 - 91,109.00 Providing consultation, health teaching and referral to support improved client outcomes within our programs. Providing virtual nursing assessments through the Collaborative Care model; these include nursing recommendations and interventions Assisting with the program development, implementation and delivery of Health Services. This includes assess and refer, health teaching, policy implementation recruitment of practitioners, and client follow up. Supporting the Nursing team as the program expands/evolves, assisting with the creation of process flows, forms, and service standards. Assisting with specialized training for the practitioners providing care within these programs and supporting those clinicians in the expectations surrounding treatment and reporting. Being responsible for scheduling assessment/treatment sessions, documenting all interactions with all stakeholders, and providing appropriate reporting updates as required by Inkblot service standards. Monitoring compliance, progress, attendance and engagement of all clients engaged in the treatment programs. Reviewing all reports received and providing timely reporting to the referring source. Development of training materials, patient resources and webinars. WHO WE'RE LOOKING FOR Qualifications Must be RN (Registered Nurse), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) Minimum 3 years of experience Registered and in good standing with Provincial College Skills & Experience Critical thinking and ability to make sound decisions in a fast-paced, dynamic setting Strong interpersonal, relationship-building, and communication skills Experience working in diverse clinical settings including community, hospital and clinics. Ability to work collaboratively with different team members and stakeholders Strong organizational skills and attention to detail related to quality of service delivery High level of proficiency with various computer programs including; Google Drive, Microsoft Office. Exigences

Candidates must be a Registered Nurse (RN), Registered Practical Nurse (RPN), or Licensed Practical Nurse (LPN) in good standing with their Provincial College. A minimum of 3 years of experience is required, along with strong interpersonal skills and proficiency in computer programs like Microsoft Office.
