In this role, you will: • Develop personalized, evidence-informed care plans to support individual rehabilitation and wellness goals. • Collaborate with a multidisciplinary team to coordinate patient-centric care and achieve optimal outcomes. • Build the next generation through mentorship and leadership (as assigned). • Stay current with best practices through continuous learning and professional development. • Utilize modern tools, including e-charting and digital communication, to streamline your workflow. • Educate patients and their families on recovery, prevention, and self-management strategies.
