   Leadership & Operations Physician Collaboration & Clinical Coordination Quality Improvement, Compliance & Medication Safety Collaboration & Stakeholder Engagement • Support the Director of Care in overseeing daily clinical operations within a retirement residence. • Provide leadership and clinical oversight for a nursing department of approximately 20 staff through recruitment, onboarding, performance management, scheduling support, and disciplinary processes. • Assume Charge Nurse responsibilities during staffing shortages to maintain continuity of resident care. • Participate in physician rounds, providing clinical updates, resident assessments, and recommendations based on ongoing observation to support care planning and clinical decision-making. • Process physician orders and coordinate timely implementation with pharmacy services. • Create, review, and approve individualized resident care plans, ensuring resident needs, interdisciplinary interventions, and regulatory requirements are accurately reflected. • Communicate physician recommendations, medication changes, and treatment plans with residents, substitute decision-makers, and family members. • Coordinate interdisciplinary care, referrals, consultations, appointments, and follow-up with hospitals, specialist clinics, pharmacies, Ontario Health atHome (OHAH), Carefor, GPCSO, rehabilitation providers, wound care, palliative care, and other community partners to ensure timely continuity of care. • Conduct weekly and bi-weekly Infection Prevention and Control (IPAC) audits of staff practices and environmental compliance. • Coordinate outbreak management by maintaining daily line lists, communicating with the facility IPAC lead and external IPAC inspectors, monitoring resident status, and implementing infection prevention measures. • Collaborate with external IPAC auditors during monthly compliance reviews and quality improvement initiatives. • Participate in annual IPAC leadership meetings focused on outbreak preparedness, respiratory illness planning, and infection prevention strategies. • Partner with the Director of Care and Public Health to coordinate annual influenza and COVID-19 immunization programs, including vaccine ordering, clinic implementation, documentation, and vaccine administration. • Coordinate narcotic destruction and medication audits in collaboration with pharmacy services to ensure safe medication practices and regulatory compliance. • Support successful Retirement Homes Regulatory Authority (RHRA) inspections by ensuring resident care plans, clinical documentation, and nursing records remain current, accurate, and compliant with regulatory standards. • Maintain accurate clinical documentation using Med e-care in accordance with organizational and regulatory standards. • Collaborate with residents, families, physicians, pharmacists, hospitals, specialist clinics, and community healthcare partners to support coordinated, resident-centred care. • Participate in recurring interdisciplinary meetings with Ontario Health atHome (OHAH), Carefor leadership, wound care specialists, palliative care teams, pharmacists, and other healthcare partners to coordinate services, resolve clinical concerns, and improve resident outcomes.
