Registered Practical Nurse, Medical & Ortho Rehab Care

As a Registered Practical Nurse on this unit, you’ll work closely with patients facing diverse medical and orthopaedic challenges, applying tailored approaches to pain management, mobility, and long-term rehabilitation. Early-career RPNs will recognize similarities in patient needs typically seen in General Medicine, while gaining added exposure to rehabilitation and orthopaedic cases that broaden practical skills. Exposure to mobility-focused care, orthopaedic recovery, and symptom management gives you a balanced mix of technical and interpersonal practice. About Medical and Orthopaedic Rehabilitation (8S) Hennick Bridgepoint Hospital’s Medical and Orthopaedic Rehab team (8 South) is built on a foundation of collaboration, where every member contributes to the delivery of patient-centered care. As a 32-bed unit with one flex bed, we care for patients transitioning from acute care with a wide range of medical and orthopaedic diagnoses. As we prioritize patient and family involvement, we continuously develop care plans tailored to the specific needs of those we serve, with a focus on reactivation, mobility, and activities of daily living. In this role you will: Practice independently and accountably within the RPN scope of practice, adhering to CNO standards and Sinai Health policies Identify changing patient needs to initiate timely consultation and collaboration with Registered Nurses and the wider interprofessional team Monitor and document the progress of clinical care plans and outcomes, adjusting strategies as needed to optimize treatment goals and patient safety Participate in the mentoring of learners, peers and other regulated and unregulated health professionals Provide health education to patients and their families using effective teaching strategies Assess spiritual, cultural and psychosocial needs of patients Provide comprehensive and accurate documentation of assessments, care provision, and evaluations Collaborate with the patient, significant others and multidisciplinary team in the provision of care Provide comprehensive and accurate documentation of assessments, care provision, evaluations, discharge and transition planning Perform other duties consistent with the job classification as required Job Requirements Mandatory Graduate of a CNO-approved Practical Nursing (PN) program Currently registered as an RPN and in good standing with the College of Nurses of Ontario (CNO) Current Basic Cardiac Life Support (BCLS) certification Preferred Experience with electronic health record (EHR) and clinical documentation Previous medical and/or surgical clinical experience Skills and Knowledge Excellent health and clinical assessment skills Demonstrated ability to build therapeutic relationships and provide patient-focused care Effective time-management and organizational skills Excellent problem solving and critical thinking skills Strong collaboration skills to build professional relationships and support integration with health care team Computer proficiency and/or basic to intermediate Microsoft Office (Word and Outlook) skills Communication skills including active listening, conflict resolution and relational practice Demonstrated leadership skills Demonstrated satisfactory job performance and attendance history required If this sounds like you and you feel ready to build your nursing career within medical and orthopaedic rehabilitation, apply now and let us know why you’d be a great addition to our team.
