They provide physical assessments, symptom control support, palliative care, rehabilitation therapy, wound care, education, counselling to individuals and families and much more. Working conditions and physical demands: This role requires a detail-oriented approach in a dynamic environment, with physical activity including lifting, carrying (using proper techniques), bending, reaching, kneeling, and other movements that emphasize good body mechanics. Individuals in the role are required to walk, sit, stand, and climb stairs throughout the day, with some tasks requiring fine hand movements. The nurse will provide holistic, person-centred care by conducting physical assessments, administering medication, and performing wound care in clients' homes and clinics. They will also collaborate with other healthcare professionals to manage chronic diseases and provide palliative and rehabilitative support.

Candidates must be licensed as a Registered Practical Nurse in the province of practice and possess a valid driver's license with access to a reliable vehicle. A satisfactory Criminal Records Check and the ability to wear Personal Protective Equipment are also required.
