Here’s a closer look at our total rewards and benefits * Reward and recognition programs, including earned-wage access through our partnership with ZayZoon * Comprehensive benefits package including Group RRSP, extended medical and dental coverage * Employee Perks and Discount Programs through our partnership with Perkopolis * Employee and Family Assistance Program * Paid orientation, training and development with opportunities to grow your career What you will do Ensures the well-being of individuals in care * Adhere to all clinical training, procedures and protocols, and implement treatment plans based on best practices as developed by the clinical team. * Responsible for supporting Direct Support Specialists in implementation of individual treatment plan. * Uses systematic methods and behavioural strategies to support individual in changing their behaviour * Implement and train on adaptive skill development and preventing and managing challenging behavior as per the Behavioural Support Plan and Individual Support Plan where appropriate * Contributes to the development of programs established by the Clinicians to teach and reinforce a variety of skills for individuals in the assigned home * Supports the Operations Manager in ensuring effective and high-quality operations in all settings * Builds strong relationships and rapport with all individuals within the home. Understands individual preferences, needs, triggers/antecedents, responses/consequences etc. * Encourage the client’s independence, choice and self- expression * Maintain knowledge of the client’s skills, abilities, interests and support needs * Act as a role model to assist the individual in the development of life skills and to promote independence whenever possible * Develops recreation plan in collaboration with Direct Support Specialists and Clinical Manager. * Develops meal plan in collaboration with Direct Support Specialists and Managers. * Ability to adapt to changing conditions and/or routine and provide creative solutions * Actively work with individuals and encourage participation while assisting the individual in their daily activities and with personal responsibilities * Coaches and monitors staff to ensure the understanding of Individual Support Plans/Plan of Care, Treatment plans and behaviour Support Plans. * Ensures operational performance concerns are escalated to Operations Manager and clinical concerns are escalated to specific clinical discipline (CM) * Responsible for adherence to meal plan and recreation plan. Ensures Effective Operations * Responsible for monitoring the functioning of all equipment in the home. (e.g., furnace, smoke detectors) * Responsible for updating and escalating home maintenance issues to the Admin and Operations Manager * Responsible for updating and escalating treatment issues to the Operations Manager and Clinical Manager in a timely manner * Support staff in scheduling system where applicable (e.g., vacations, sick calls, etc.) and communication with coordination when on call. * Conducts fire drills in the homes based on licensing requirement and ensures staff sign off * Identified hazards are forwarded to the Admin in the form of a maintenance request or work order. Escalation to the Operations Manager for serious concerns or hazards * Ensures services in community settings (e.g. leisure) are person-centered and conducive to goal attainment and safe for service delivery in collaboration with Operations Manager and/or Activity Coordinator (or other applicable team members). * Supports the OM and HR to ensure employees are supported with modified work activities where applicable (ie. WCB) * Ensures that the home is clean at all times and cleaning/sanitation checklists are signed off on * Responsible for emergency scheduling changes of Direct Support Specialists Administration and Documentation * Work with individual and guardian to establish goals for ISP with support of CM, including preparing program materials for both BSP and ISP * Responsible to track individual progress through the collection of data (and graphing as applicable) and to support Direct Support Specialists in collection of data * Documents/records exceptional circumstances, including Incident Reports within funder guidelines (medical or behavioural), completes behaviour tracking logs, and tracks multiple behaviours * Assists in conducting baseline measurements and other indirect measures as instructed by the Clinicians * Supporting Operation Manager and/or Admin with all aspects of house management (e.g. inventory, grocery, maintenance) * Regular auditing of individual documentation. Track and identify changes in individual behaviour and overall health status (including MAR), and escalate issues to appropriate Manager * Set up, attend and take minutes for home or team meetings in collaboration with Operations Manager * Responsible for day to day management of petty cash/comfort funds for the home. * 100% of all staff sign in and out are accounted for (supporting Operations Manager) * 100% of incident reports are complete * 100% of individual documentations are complete What you will need to be successful * Bachelor’s degree, diploma or advanced certification in relevant area. * 1-2 years relevant experience * Current CPR and First Aid Certification. * Clear Criminal Record Check * Fluent English Skills in verbal, reading and written communication that meets the job requirement. * Proficiency in Microsoft Office suite, and the ability to learn new programs specific to the business. * Must have a valid Driver's license and be able to bring a vehicle to work each day as you will be travelling between homes. Exigences
Candidates must possess a bachelor's degree, diploma, or advanced certification in a relevant field along with 1-2 years of experience. A valid driver's license, vehicle, and current CPR/First Aid certification are mandatory for this role.
